Enrollment Policies and Procedures

The following information will need to be completed by families enrolling in PCN CDC and need to be turned in two weeks prior to a child’s start date:

  • A packet of forms will be provided by PCN CDC and must be completed.
  • The PCN CDC Parent Handbook must be read and understood.
  • The $50.00 registration fee per family must be submitted by check or money order
    with enrollment papers.
  • The first week’s fee per child must be submitted by check or money order with contract.

A copy of the accepted Contract will be issued for your files upon receipt per parents request.
